To access your 2025 W-2: visit www.mytaxform.com and follow the instructions if:
- Your last day with H‑E‑B was before January 15, 2026, OR
- You elected to receive a printed copy of your W-2
To access your 2025 W-2: visit www.mytaxform.com and follow the instructions IF:
- Your last day with H‑E‑B was before January 15, 2026, OR
- You elected to receive a printed copy of your W-2
- Enter H‑E‑B Employer Name/Code: 13054 (No need to add your name, just the code)
- Click on "Register Now" to start the process or enter your User ID if you already have an account (your User ID may be your Peoplesoft ID). If you don’t remember your user ID, click on” Register Now” and fill out the information; it may show your user ID.
- Enter your information in required fields to validate your identity.
- If you are creating an account, once you've logged in, you will be prompted to create a personalized username.
- Follow the instructions to consent for electronic delivery. This consent will only apply to the current W-2 year.
- View, download, or print a copy of your W-2 anytime or import your W-2 information directly to your tax prep software.
- Enter H‑E‑B Employer Name/Code: 13054 (No need to add your name, just the code)

New Tax Provision: Tips & Overtime
Effective 2025 through 2028, you may be able to reduce your taxable income under the One Big Beautiful Bill Act (OBBBA) by deducting qualified tips and overtime (defined as the “half” portion of “time-and-a-half” compensation), up to the limits defined by the law. This deduction is available whether you itemize or take the standard deduction. You will find this information in Box 14 on your Form W-2:

What is Form 1095-C?
Form 1095-C is a tax form used to report information to taxpayers and the IRS about minimum essential health insurance coverage. Employers like HEB who provide minimum essential coverage (health insurance that meets basic requirements) must report this coverage to the IRS. If you request proof of your coverage, employers are required to provide it within 30 days.
Note: The Form 1095-C is no longer required for tax filing, and you can complete your taxes without it.

How can you request your Form 1095-C?
If you would like your form electronically:
- Visit https://www.eyacaforms.com/HEB.
- Click on “Register as New User.”
- Complete the form and click “Create New Account.”
- You will immediately receive a confirmation email from EY ACA Form requiring you to verify your account. Click the link in the email and opt in to e-forms. The confirmation link will expire in 24 hours.
- If you elect electronic delivery, you will receive an email from EY ACA Form when your form is available to download (within 30 days).

States with Individual Insurance Mandates, such as CA, NJ, RI & DC Employees living in states with laws that require reporting of health coverage will continue to receive a paper copy of the Form 1095-C for state filing tax purposes. Employees filing taxes in one of these states are encouraged to retain a copy of the form for their state tax records.
